We are seeking an Adult In Community Health Clinic
We previously had a in a community medical clinic for our adults born with Down syndrome. Unfortunately, that clinic is no longer accepting new patients. However, we are actively working to secure a new Primary Care Network (PCN) in Edmonton where we can refer adults ( 18+) with Down syndrome for ongoing medical support.
If you are aware of any doctors or nurse practitioners who may be interested in supporting this population, please don’t hesitate to let us know.
We also have copies of the Global Down Syndrome Foundation Medical Care Guidelines for Adults with Down Syndrome, which we are happy to share with families to pass along to their physicians. If you would like a copy, please email us at julie@edss.ca, and we will be happy to provide it.
What is a Primary Care Network?
Primary Care Networks (PCNs) are a made-in-Alberta approach to improve access to, and better co-ordinate, primary care services (the first point of care most people seek when they have a health issue) for Albertans.
In a PCN, family doctors and teams of health professionals work collaboratively to increase Albertans’ access for care. These teams can include nurses, dieticians, social workers, mental health specialists, pharmacists and many more.
